Please download the reference form and return it to Debbie Comery (Post 16 Receptionist) via post at; Djanogly City Academy Nottingham, 16 - 19 Centre, Sherwood Rise, Nottingham Road, Nottingham, NG7 7AR or via fax on 0115 9424034. Your reference should be filled in by your Head of Year or your Deputy Head at your school.
